Telenotarzt becomes a lifesaver in Aachen

The German emergency medical service is increasingly suffering from a shortage of doctors. Especially in rural areas, it can take a while for an emergency doctor to arrive at the site. The ambulance service in Aachen therefore relies on the telenotist – until the help is on site, the paramedics are accompanied by an emergency doctor telemedicine.
Since April 2014, the telenotist has been supporting the ambulance service in Aachen around the clock and accompanied the paramedics by mobile phone. The paramedics are smart health connected to the emergency doctor in the control center via their headset, who is sitting in a specially equipped workplace – the telenotarzt center. From there, he coordinates the deployment of the paramedics and initiates initial medical care for the patients. This shortens the time until the first doctor contact (so-called therapy-free interval) and increases the quality of patient care. Especially when treating cardiovascular emergencies, rapid diagnosis and treatment of the patient is extremely important in order to prevent life-threatening heart muscle damage.
The innovative service is developed and operated by P3 telehealthcare GmbH, a joint venture between P3 communications GmbH and Docs in Cloud GmbH,
Telenotarzt: That's how it works
The deployment of the paramedics is monitored and accompanied by experienced emergency physicians on site from the Telenotarzt headquarters. The telenotists maintain constant contact with the emergency services on site via mobile radio. All vital parameters such as ECG, pulse oximetry and blood pressure, photos of the site and ehealth streams are transmitted live from the ambulance, so that the telenotist is informed at all times about what is happening on site. This is made possible by a mobile monitoring unit, the so-called peeqBOX. The transmitted data enable the telenotist to make a diagnosis and advise the rescue team on site regarding the necessary treatment steps. Software also supports the implementation of guideline-compliant patient care and thus enables uniform, high-quality care on site.
At the telenotist, the ambulance becomes a high-tech car
There is a permanently installed communication unit in the ambulance, which has increased performance thanks to special antennas on the vehicle roof and additional data cards. It provides a secure WLAN for data exchange with the mobile monitoring unit. The other devices in the ambulance are smart health connected to the integrated communication unit via a wired network. In addition: a ehealth camera that can be controlled by the telenotist is installed on the ceiling of the ambulance and can be used to accompany the transport. The ehealth data is not saved, but is transmitted to the telenotarzt headquarters via live stream.

Telemedicine is proving itself more and more in emergencies
In many areas of medicine, telemedical networking between medical staff and specialists has improved the quality of care. For the treatment of acute strokes, telemedicine systems between hospitals with a stroke unit and hospitals that do not have such a station have been used for several years. Here, telemedical networking, including ehealth chatting in real time, has proven to be more advantageous than a pure telephone call. In emergency medicine, the benefits of transmitting the 12-lead ECG to a cardiologist during a heart attack have been proven. Telemedicine is not intended to replace physician contact, but to help relieve existing physicians and close gaps in care. Already 1500 emergency operations could be successfully accompanied with the telenotist.
In view of the consistently positive results, P3 telehealthcare GmbH would like to work out a solution together with the cost bearers in order to offer the service to other emergency services quickly and on a large scale
